Our Philosophy: Growth through Collaboration & Partnership

we recognize that creating meaningful change requires collaboration and partnership with a wide range of stakeholders. We understand that no single organization or individual can address the complex needs of young adults with disabilities alone. Therefore, we actively engage with clients, families, caregivers, community organizations, educational institutions, employers, and other stakeholders to ensure that our Youth Transition Program meets the diverse needs of participants and provides comprehensive support for their transition to adulthood.

Family-Centered Approach:

We believe that families are essential partners in the transition process and play a crucial role in supporting young adults with disabilities as they navigate the challenges of adulthood. Our program takes a family-centered approach, which involves actively involving families in the transition planning process, providing them with information, resources, and support, and collaborating with them to identify goals and priorities for their loved ones.

Community Partnerships:

We understand the importance of connecting participants with community resources, support networks, and opportunities for social inclusion and engagement. To facilitate this, we have established partnerships with local community organizations, service providers, recreational facilities, and businesses to provide participants with access to a wide range of community-based activities, services, and opportunities for social interaction.

Educational Partnerships:

Education is a key component of the transition process, and we partner with educational institutions, vocational training programs, and post-secondary institutions to support participants in accessing educational opportunities that align with their interests and goals. Our partnerships with schools and colleges allow us to provide participants with information, guidance, and support in navigating the admissions process, accessing accommodations and support services, and transitioning successfully to post-secondary education.

Employment Partnerships:

Securing meaningful employment is a critical step in the transition to adulthood, and we collaborate with employers, businesses, and workforce development agencies to create opportunities for participants to gain work experience, develop job skills, and explore career pathways. Our partnerships with employers allow us to facilitate job shadowing, internships, and employment placements, as well as provide participants with support in job search, resume writing, interview preparation, and workplace accommodations.

Advocacy and Policy Partnerships:

We are committed to advocating for policies and practices that promote inclusivity, accessibility, and equity for individuals with disabilities. To advance this goal, we collaborate with advocacy organizations, disability rights groups, and policymakers to raise awareness about the needs of young adults with disabilities, advocate for systemic change, and influence policy decisions that impact their lives.
